New Students, once logged in you can complete all REQUIRED Health Forms (even from your phone!):

• Complete Health History Questionnaire
• Complete TB Screening Questionnaire
• Enter dates of your Immunizations
• Upload a signed copy of your Immunization Record - take a picture, then upload
• Upload a copy of your latest Physical Exam (within past 18 months) - take a picture, then upload

All Students will be able to access:

• Secure communication with the Health Center
• Secure communication with the Counseling Center
• Lab results
• Educational health information
• Update insurance information
